Musculus gluteus medius
The largest muscle in the horse. Origin: Longissimus lumborum, gluteal surface of ilium, sacrum, sacroiliac and sacrosciatic ligaments. Insertion: Greater trochanter. Innervation: Cranial gluteal nerve. Function: Extends hip joint; abducts hindlimb. The principal extensor of the hip and a major propulsive muscle.
Origin: Gluteal surface of the ilium, gluteal fascia, and sacroiliac ligament
Insertion: Greater trochanter of the femur (trochanteric fossa)
Action: Extends the hip joint; primary driver of propulsion; abducts the hindlimb
Innervation: Cranial gluteal nerve (L5-S2)
The gluteus medius is the largest muscle in the horse and the principal extensor of the hip joint. 'Hunter's bumps' (subluxation of the sacroiliac joint) can cause asymmetric atrophy of the gluteal muscles. The trochanteric bursa deep to the accessory gluteal tendon can become inflamed (trochanteric bursitis).
The gluteus medius is commonly affected during exertional rhabdomyolysis (tying-up), where muscle fibers break down during or after exercise. Can be sporadic or recurrent (PSSM, RER).
The gluteus medius inserts near the SI joint. Overexertion causes SI ligament damage, a common cause of poor hindquarter performance.
Any chronic hindlimb issue causes progressive gluteal wasting as the horse offloads the affected limb.
The gluteus medius is commonly affected during exertional rhabdomyolysis (tying-up), where muscle fibers break down during or after exercise. Can be sporadic or recurrent (PSSM, RER). Signs include: Firm, painful hindquarters during or after exercise; shortened stride; reluctance to move forward; excessive sweating; dark (brown/red) urine from myoglobin; elevated CK/AST on bloodwork.

Signs include: Asymmetric hip height (hunter's bump — prominent tuber sacrale); difficulty engaging hindquarters; reluctance to canter on one lead; poor impulsion; pain on palpation over the sacroiliac region.
Any chronic hindlimb issue causes progressive gluteal wasting as the horse offloads the affected limb. Signs include: Visibly smaller hindquarter on the affected side (view from behind); asymmetric muscling; persistent mild hindlimb lameness; reduced power on the affected side.
