Musculus multifidus
The most important deep spinal stabilizer muscle. Spans 2-4 vertebral segments. Research by Dr. Kevin Haussler and others has shown multifidus atrophy is closely correlated with back pain and is the primary target for equine core rehabilitation programs. Can be assessed with ultrasound.
Origin: Articular and transverse processes of vertebrae
Insertion: Spinous processes of vertebrae 2-4 segments cranially
Action: Fine segmental stabilization of the spine; proprioceptive feedback; extends individual segments
Innervation: Medial branches of dorsal rami of spinal nerves (segmental)

Targeted exercises (belly lifts, carrot stretches, tail pulls, backing, raised poles) specifically activate the multifidus and are now standard in equine physiotherapy.

Signs include: Visible atrophy alongside specific spinous processes (may be asymmetric); poor topline unresponsive to feeding or general exercise; chronic recurring back pain.

Signs include: Horses with multifidus weakness show: poor topline, hollow back under saddle, difficulty maintaining collection, recurring back soreness.
