Musculus supraspinatus
Fills the supraspinous fossa above the scapular spine. Extends the shoulder and prevents it from collapsing during weight bearing. One of the two muscles affected in 'Sweeney' (suprascapular neuropathy).
Origin: Supraspinous fossa of the scapula
Insertion: Greater and lesser tubercles of the humerus
Action: Extends the shoulder; stabilizes the shoulder during weight bearing
Innervation: Suprascapular nerve (C6-C7)
Suprascapular nerve damage (from trauma to the cranial border of the scapula) can cause atrophy of the supraspinatus and infraspinatus muscles, known as 'Sweeney syndrome'. Clinical signs include flattening of the shoulder region and instability of the shoulder joint.
Damage to the suprascapular nerve from collision with doorframes, posts, or fences causes rapid atrophy of both the supraspinatus and infraspinatus. Named 'Sweeney' historically.
Chronic strain or degeneration of the supraspinatus insertion, increasingly recognized with ultrasound as a cause of forelimb lameness.

Signs include: Rapid muscle wasting above and below the scapular spine; prominent scapular spine visible from the side; shoulder pops outward (lateral instability) during weight bearing; stumbling; circumducting gait.

Signs include: Intermittent forelimb lameness; pain on direct palpation over the point of the shoulder; reduced stride length; lameness worse on hard ground; may respond to ultrasound-guided injection.
