Capillary Refill Time: How to Check Your Horse's Circulation
If you own a horse, you need to know how to check capillary refill time. Full stop. It takes about three seconds to perform, requires zero equipment, and can tell you more about your horse's circulatory status than almost any other quick assessment you can do in the field. When you're standing in a pasture at midnight wondering whether to call the vet, CRT is one of the things that helps you make that decision.
What Capillary Refill Time Actually Tells You
CRT measures how quickly blood returns to the tiny capillary beds after being briefly displaced. When you press on the gum tissue, you squeeze blood out of the capillaries in that area, blanching the tissue white (or pale). When you release the pressure, blood flows back in. How fast it returns reflects the state of peripheral perfusion, basically, how well the heart and blood vessels are delivering blood to the tissues.
It's a window into cardiovascular function. Normal CRT means the heart is pumping adequately, blood volume is sufficient, blood vessels are maintaining appropriate tone, and the tissues are being perfused. Abnormal CRT can indicate dehydration, shock, cardiovascular compromise, endotoxemia, or severe pain, all situations where you want to know what's happening sooner rather than later.
How to Check It
The technique is almost embarrassingly simple:
- Lift your horse's upper lip. You want to see the gum tissue above the upper incisors. Most horses tolerate this easily, though some will need a moment to stop being offended about it.
- Press firmly with your thumb or finger. Apply enough pressure to blanch the tissue; you should see a white or pale spot where your thumb was when you release. Press for about 2 seconds.
- Release and count. Watch the blanched spot and count how many seconds it takes for normal pink color to return.
That's it. Three steps. Practice on your horse when they're healthy and relaxed so you know what their normal looks like. Every horse is a little different, and knowing the baseline makes abnormalities much easier to spot.
Getting Your Horse Used to It
Some horses object to having their lip lifted. Dramatically. Eyes wide, head flying up like you've asked them to do something deeply personal. Fair enough. But you need to be able to check gums in an emergency, so desensitize your horse to this when things are calm.
Start by just touching the muzzle. Rub around the lips. Gradually work up to lifting the upper lip gently, praising when the horse stands still. Do it during grooming sessions, after feeding, whenever the horse is relaxed and cooperative. Within a week most horses stop caring entirely. The ones that still fuss might need a buddy holding a lead rope while you work, or a treat immediately after. Whatever it takes, because fumbling with a panicking horse's mouth at 2 AM while also panicking yourself is a scenario nobody enjoys.
What's Normal
Normal capillary refill time in horses is less than 2 seconds. Most healthy, well-hydrated horses at rest will refill in about 1 to 1.5 seconds. The color returns smoothly and evenly.
While you're up there looking at gums, note the color too. Normal gum color is a healthy salmon pink. This is part of your baseline assessment; gum color and CRT together give you a much more complete picture than either one alone.
A few variables affect CRT in perfectly healthy horses. Cold weather can slow peripheral circulation slightly, adding a half-second to refill. Heavy exercise temporarily changes things. A horse that just galloped around the field may have slightly faster CRT from elevated cardiac output and vasodilation. These normal fluctuations are exactly why baseline knowledge matters so much. You need to know what your horse's "normal after a gallop" looks like versus "normal standing in the crossties on a Tuesday."
Abnormal Findings and What They Mean
Prolonged CRT (Greater Than 2.5 Seconds)
This is the most common abnormality you'll encounter. A CRT of 3 seconds or more suggests reduced peripheral perfusion. The causes include:
- Dehydration - Reduced blood volume means less pressure to push blood into capillary beds. This is common in horses that haven't been drinking adequately, have been sweating heavily without fluid replacement, or have had diarrhea.
- Shock - Whether from blood loss, severe infection, or endotoxemia, shock reduces peripheral perfusion as the body prioritizes blood flow to vital organs.
- Severe pain - Intense pain (such as from colic) causes sympathetic nervous system activation, which constricts peripheral blood vessels and can prolong CRT.
- Heart failure - Reduced cardiac output means reduced perfusion everywhere.
Very Rapid CRT (Less Than 1 Second)
Counterintuitively, a very fast CRT can also be abnormal. In early endotoxemia or sepsis, blood vessels dilate excessively, and capillaries overfill. The refill appears almost instantaneous; the blanched spot fills before you've even finished lifting your thumb. This often accompanies a "brick red" or injected gum color. If you see hyperemic (dark red or purple) gums with nearly instant CRT, that horse needs a vet immediately. This is a sign of serious systemic inflammation.
I once checked a horse whose gums refilled so fast it was almost like pressing into a ripe tomato. The color just rushed back in a wave. His gums were the color of raw steak. The vet was there within forty minutes, and that horse spent three days on IV fluids fighting an enteritis that could have killed him. Smart horse people never evaluate CRT in isolation. It's one instrument in an orchestra of vital signs, and the whole ensemble tells the story. When you check CRT, you should also be checking:
- Heart rate. Normal resting is 28 to 44 beats per minute. A stethoscope behind the left elbow, or fingers pressed against the facial artery under the jaw. Elevated heart rate combined with prolonged CRT screams cardiovascular compromise.
- Respiratory rate. Count flank movements for 15 seconds and multiply by four. Normal is 8 to 16 breaths per minute at rest.
- Rectal temperature. Normal is 99.0 to 101.5 degrees Fahrenheit. Fever combined with abnormal CRT points toward infection or systemic inflammation.
- Gut sounds. Press your ear or a stethoscope to the flank in all four quadrants. Silence is concerning. Excessive rushing or tinkling sounds also warrant attention. Understanding how the hindgut functions helps you interpret what you're hearing.
- Skin turgor. Pinch the skin on the neck; it should snap back within two seconds. Tenting suggests dehydration, which correlates with prolonged CRT.
Together, these five assessments create a snapshot of your horse's physiological status that your vet can use to triage over the phone. "His heart rate is 64, CRT is four seconds, gums are dark red, and I'm hearing almost nothing in the right lower quadrant" paints a radically different picture than "he seems off."
When to Worry
CRT is never evaluated in isolation. It's one vital sign among several. But here's a practical guide for when CRT findings should trigger action:
Call your vet if:
- CRT is consistently over 3 seconds
- Gums are any color other than pink (especially dark red, purple, or white)
- CRT is abnormal AND the horse shows other signs of distress (elevated heart rate, pawing, rolling, sweating, not eating)
- You see a toxic line along the gum margin
Monitor closely if:
- CRT is 2.5 to 3 seconds but the horse is otherwise acting normal (recheck in 15 to 30 minutes)
- It's very hot and the horse may just be mildly dehydrated (offer water and recheck)
Probably fine:
- CRT is just over 2 seconds on a hot day when the horse has been exercising (normal physiological response)
- Gums are pink and horse is bright, alert, eating, and drinking normally
Teaching It to Others
Don't hoard this knowledge. Every person who interacts with your horses should know how to check CRT and gum color. Barn staff, working students, the teenager who feeds on weekends, your spouse who "doesn't do horses" but might be the only one home when something goes wrong.
Make it a barn policy. Print a laminated card with normal ranges and hang it in the feed room. Practice at a barn meeting. The more eyes that know how to assess basic vitals, the faster problems get identified. CRT combined with temperature, heart rate, respiratory rate, and gut sounds forms a basic vital signs assessment that every horse person should be able to perform. Your vet will also appreciate getting useful information when you call, rather than "my horse just doesn't look right."
