Dehydration Assessment in Horses: Skin Tent Test and Beyond
Horses are roughly 60 to 70 percent water. That's a staggering amount of fluid to maintain, and it means even mild dehydration can start hammering organ function, gut motility, and overall performance faster than most people realize. A 1,000-pound horse that's 5 percent dehydrated has lost about 25 liters of fluid. Picture ten and a half two-liter soda bottles emptied out on the barn floor. That's not a subtle deficit.
The tricky part is that horses don't always broadcast trouble. They're stoic prey animals hardwired to mask vulnerability. By the time a horse looks clearly dehydrated to the casual observer, you're probably dealing with a situation that demands veterinary intervention, IV fluid lines, and a hefty invoice. Learning to catch it early, knowing what your fingers and eyes should be looking for, is one of those essential horsemanship skills that pays for itself a hundred times over. According to the Texas A&M Veterinary Medical Teaching Hospital, subclinical dehydration is one of the most commonly missed problems in equine management, particularly during winter months when owners assume their horses are drinking enough simply because it isn't hot outside.
The Skin Turgor Test (Pinch Test)
This is the one everybody knows, and for good reason. Quick. Requires zero equipment. Gives you useful information on the spot. Pinch a fold of skin on the horse's neck or shoulder, pull it gently away from the body, then release. In a well-hydrated horse, the skin snaps back flat almost instantly, within one to two seconds. Smooth, elastic, immediate.
If the skin stays tented for two to four seconds, you're looking at roughly 5 to 6 percent dehydration. If it takes longer than four seconds, or the fold just sits there like a little tent refusing to flatten, that's 8 percent or more and you're in serious territory. The Merck Veterinary Manual puts it even more bluntly: skin tenting persisting beyond five seconds typically indicates a horse requiring intravenous fluid therapy, not just access to a water trough and crossed fingers.
A few caveats, though. Older horses often have decreased skin elasticity regardless of hydration status. Their skin loses that rubbery snap the way all aging connective tissue does, so a 25-year-old gelding might show prolonged tenting even when his water intake is perfectly fine. Very fit horses or those in excellent body condition sometimes show less dramatic skin tenting because subcutaneous fat keeps things taut. And where you pinch matters more than people think. The point of the shoulder tends to be more reliable than the eyelid, which some old-school texts recommend, because it's less affected by individual anatomical variation.
Cornell's equine hospital team recommends performing the pinch test in the same location each time you check so you develop a baseline for that specific horse. What looks normal on a 17-hand Warmblood might read differently on a fine-skinned Thoroughbred. Know your horse's normal, and departures from it become obvious.
Bottom line: skin turgor is a useful screening tool, not a definitive diagnosis. Use it alongside other indicators. One test alone tells you part of the story. Multiple tests together paint the whole picture.
Capillary Refill Time (CRT)
Capillary refill time tells you about peripheral perfusion, how effectively blood is circulating out to the tissues. Lift the horse's upper lip and press your thumb firmly against the gum above the incisors for two seconds. Release. You'll see a white blanched spot where you pressed. Time how long it takes for the pink to flood back.
Normal CRT is less than two seconds. The color returns almost as fast as you can register it happening. A CRT of two to three seconds suggests compromised circulation, which could mean dehydration, pain, or the earliest stages of shock creeping in. Greater than three seconds is a genuine red flag indicating significant cardiovascular compromise, and you should be picking up the phone.
CRT is particularly valuable because it reflects what's happening systemically, not just locally. A dehydrated horse has reduced blood volume, which means reduced perfusion pressure, which means slower capillary refill. The AAEP notes that CRT combined with skin turgor gives a far more accurate hydration picture than either assessment performed alone. But keep in mind that CRT can also be prolonged by shock from causes unrelated to dehydration, severe pain, cold ambient temperatures that constrict peripheral vessels, or even the horse clenching against you while you've got its lip flipped up. Context always matters.
Mucous Membrane Color
While you've got that lip up checking CRT, look at the gum color. Really look. Normal equine gums are a healthy salmon pink, moist, glistening, the kind of color that doesn't make you think twice. Dehydration changes this picture in several distinct ways, and each variation points somewhere different.
Pale pink or white gums suggest poor perfusion and possible anemia. Dark red or "injected" gums, where you can almost see individual capillaries standing out like tiny rivers on a map, indicate endotoxemia or severe dehydration. A toxic line, that dark purple-red band along the gum margin right above the teeth, is an ominous sign of endotoxic shock that should make your stomach drop. Yellowish gums suggest liver involvement, a whole separate conversation.
Also note the moisture of the membranes themselves. Dry, tacky gums are a straightforward sign that the horse doesn't have enough fluid on board. Normal gums should feel slick and wet when you run your finger across them, like the inside of your own cheek. If your finger drags or the gums feel sticky, almost like double-sided tape, that's abnormal. UC Davis researchers have noted that mucous membrane tackiness often appears before skin turgor changes become obvious, making it potentially the earliest palpable indicator of developing dehydration in horses.
Other Signs of Dehydration
Sunken Eyes
In moderate to severe dehydration, the fat pad behind the eye loses fluid volume and the eyes appear to sink into the skull. This is more obvious in lean horses and foals, where there's less cushioning tissue to start with. If you can clearly see a gap between the eyeball and the upper eyelid, the horse is significantly dehydrated, probably 7 to 8 percent or more. By this point you shouldn't need convincing to call the vet. The horse looks sick. Trust what you see.
Dry Fecal Balls
Check the manure. Seriously, get in the habit. A well-hydrated horse produces fecal balls that break apart easily and have visible moisture content. You can see the plant fiber glistening. Dry, hard, small fecal balls that look like they've been sitting in the sun for three days, even though they're fresh, suggest the colon is reabsorbing more water than usual. That's a compensatory mechanism kicking in when systemic hydration drops, and it dramatically increases the risk of impaction colic. The relationship between dehydration and colic isn't casual. It's causal. Texas A&M's internal medicine department lists dehydration as one of the top three predisposing factors for large colon impactions.
Decreased Urination
If the stall is unusually dry or you haven't seen the horse urinate at its normal frequency, that's worth noting in your mental checklist. The kidneys conserve water aggressively during dehydration, producing smaller volumes of more concentrated, darker urine. A horse that normally soaks half its stall by morning and suddenly isn't? Pay attention.
Elevated Heart Rate
The heart compensates for reduced blood volume by beating faster, trying to maintain adequate circulation with less fluid to push around. A resting heart rate consistently above 40 bpm in an adult horse standing quietly in its stall warrants attention. In the context of other dehydration signs, tachycardia helps confirm the clinical picture and gives your vet useful triage information over the phone.
Depression and Decreased Appetite
A dehydrated horse often becomes lethargic. Dull eyes. Slow to respond. Standing in the back of the stall like it wants to be left alone. Decreased interest in feed, especially grain, which most horses are embarrassingly enthusiastic about, is a nonspecific but important sign. Horses know when they don't feel right, and they withdraw. Listen to them when they go quiet on you.
The Percentage Scale
Veterinarians typically estimate dehydration as a percentage of body weight lost as fluid. The Merck Veterinary Manual and AAEP both use variations of this scale in clinical practice:
- Less than 5% - Clinically undetectable by physical exam. The horse might drink a little less enthusiastically but shows no outward signs. This is where most subclinical dehydration lives, especially in winter when cold water discourages intake. Insidious, because you can't see it happening.
- 5 to 6% - Mild. Skin turgor slightly prolonged, two to four seconds. Gums may be slightly tacky. The horse is still functional but starting to show cracks in the facade.
- 7 to 8% - Moderate. Skin turgor obviously prolonged. Eyes may appear sunken. CRT approaches 3 seconds. Heart rate creeping up. The horse is visibly unwell and anyone paying attention can see it.
- 9 to 10% - Severe. Skin tenting persists for many seconds. Gums are dry and may be discolored. CRT greater than 4 seconds. Eyes clearly sunken. This horse needs emergency IV fluids immediately, not in an hour, not after you finish the other chores. Now.
- Greater than 12% - Life-threatening. Organ failure and cardiovascular collapse. Without aggressive treatment, death is likely, and even with treatment the prognosis can be guarded depending on the underlying cause.
When It Becomes an Emergency
Call your vet right away if you observe any of the following:
- Skin turgor greater than 4 seconds
- CRT greater than 3 seconds
- Gums that are dry, dark, or discolored
- Sunken eyes
- Heart rate above 50 at rest combined with other signs
- Horse has not been drinking for more than 12 hours
- Dehydration accompanied by colic signs, diarrhea, or fever
Dehydration combined with diarrhea is an especially dangerous combination because the horse is hemorrhaging fluid faster than any compensatory mechanism can keep up. Colitis cases can become critically dehydrated within hours, not days. Similarly, a horse that's dehydrated and colicking is at extreme risk for impaction, and the dehydration itself may be what triggered the colic in the first place, creating a vicious feedback loop that only IV fluids and veterinary intervention can break.
Preventing Dehydration
Most dehydration in domestic horses is preventable. Clean, palatable water available at all times is non-negotiable, and "available" means the horse will actually drink it. Algae-coated troughs, water that tastes like rubber hose in August, buckets frozen solid in January: horses notice all of it and adjust their intake accordingly. They're picky drinkers, far more so than most owners give them credit for.
In winter, consider heated water buckets or stock tank heaters. Research from multiple universities, including UC Davis and Penn State, consistently shows horses drink significantly more when water temperatures sit between 45 and 65 degrees Fahrenheit. Some horses will reduce their water intake by 40 percent or more when offered near-freezing water. Forty percent. That alone can push a horse into subclinical dehydration over the course of a single cold week.
Electrolyte supplementation after heavy exercise or during hot weather can encourage drinking and replace what's lost in sweat. Equine sweat is hypertonic, meaning horses lose substantially more sodium and chloride per liter of sweat than humans do. A tablespoon of plain loose salt in the daily grain, or a salt block always available in the stall and the paddock, provides baseline sodium that stimulates the thirst response. Some horses won't lick a block enough to meet their needs, so top-dressing the feed is insurance.
Soaked hay, beet pulp mashes, and wet feeds are excellent ways to sneak extra water into a horse that's a reluctant drinker. A horse eating 20 pounds of dry hay gets essentially zero water from its forage. That same horse eating soaked hay cubes or a beet pulp slurry might take in several extra liters without any effort or argument. It's one of the simplest management tools available and criminally underused.
Pay extra attention to horses during transport, competition, illness, and extreme weather, both scorching heat and bitter cold. These are the situations where dehydration sneaks up fast, compounds quickly, and catches people off guard. And get in the habit of doing quick assessments regularly. Pinch the skin, check the gums, glance at the manure. It takes thirty seconds and it keeps you calibrated to what's normal for your individual horse. That familiarity, that baseline awareness, is worth more than any single diagnostic test when the day comes that something changes.

A horse at rest in moderate weather needs roughly 5 to 10 gallons (20 to 40 liters) per day. Horses in work, lactating mares, and horses in hot or humid conditions can need 15 to 20 gallons or more. Water intake also increases when eating dry hay versus pasture. The most reliable approach is providing constant access to clean, palatable water and monitoring consumption rather than trying to hit a specific number.
What does the skin pinch test tell you?
It estimates the degree of dehydration based on skin elasticity. A well-hydrated horse's skin snaps back immediately (within 1 to 2 seconds). A delay of 3 to 4 seconds suggests 5 to 6% dehydration. A delay of 6 to 10 seconds suggests 8% or more, which is serious. However, the test is less reliable in older horses (who have decreased skin elasticity regardless of hydration), obese horses, and very thin horses. Use it alongside CRT and gum assessment, never alone.
When is dehydration an emergency in horses?
Any dehydration over 5% warrants veterinary attention. Signs of severe dehydration include a skin pinch that stays tented for more than 4 seconds, capillary refill over 3 seconds, dry or tacky gums, sunken eyes, elevated heart rate above 48 bpm, and dark concentrated urine. At 8 to 10% dehydration, horses are at risk of organ damage and circulatory collapse. Intravenous fluid therapy is the only effective treatment at that stage.
Why won't my horse drink enough water?
Common reasons include water that is too cold, too warm, unfamiliar-tasting (common when traveling), stale, or located near something the horse finds threatening. Some horses reduce drinking when stressed. Adding a small amount of salt to the daily feed (1 to 2 tablespoons of loose table salt) encourages drinking. Flavoring water with a splash of apple juice can help horses accept unfamiliar water sources when away from home. Always offer unflavored water alongside flavored water so the horse has a choice.
